Expert Session – External Evaluation – 13th October 2025 – 13:45-15:15 BST – Room: Ballroom 4
1)Are healthcare regulators and accreditors having trouble keeping up? There is always a lag between establishing regulations and standards and changes in health systems. Healthcare is becoming more complex and changes are accelerating. Many argue for a system-based approach to patient safety improvement but in practice we still see a linear logic not taking the complexity into account. Individuals are sanctioned and blamed because what is considered objective evidence of healthcare practice is assessed against compliance to procedures close to the patient where the active failure occurred. This needs to change and we must rethink how regulation and accreditation contributes to promoting patient safety improvement, accountability and learning health systems and where innovation is needed. Session participants will: 1) Increase knowledge about the role of regulation and accreditation in patient safety from different perspectives (research, patient, regulation, accreditation, clinical) 2) Learn about systems vs individual responsibility logic in patient safety from different countries 3) Participate in chaired panel/group discussions about how different regulation/accreditation practices contributes to system improvement or allocating blame and suggest new ways of innovating the field.
